x^4+4x^2y^2+16y^2

this started to look like a perfect square

x^4 and 16 y^2

but the middle term should be 8x^2y

so you need to use quadratic equation a= 1, b = 4, c = 16

did you copy this correctly?

should it be x^4 + 8x^2y^2 + 16y^4
