x1 = 1/4 y1 = 3/4 x2 = 0 y2 = 5/4

Starting with point slope form which is

y - y1 = m(x - x1)

we can write this as

y - 3/4 = ((5/4 - 3/4)/(0 - 1/4))(x - 1/4)

y - 3/4 = ((1/2)/(1/4))(x - 1/4)

y - 3/4 = 1/2*4/2(x - 1/4)

y = x - 1/4 + 3/4

y = x + 1/2 which is in the slope-intercept form y = mx + b

where slope m = 1 and y-intercept = 1/2
