form a quadratic equation whose zero is -4

A quadratic equation has 2 roots.

If the only zero is x = -4, then the equation has double roots, both equal to x = -4.

i.e. the quadratic equation is (x + 4)^2 = 0, that is,

x^2 + 8x + 16 = 0
